Clinical  Practicum for a Home Visit

背景：患者尤先生，72岁，因糖尿病半年前曾在我院住院治疗。住院期间参加了

糖尿病教育课程两次，基本掌握了糖尿病自我保健的一般知识。住院期间血糖控

制在7.2~7.5mmol/L。出院前已掌握胰岛素的正确使用及低血糖防范。现出院已半

年，护士王小姐带两位护理系学生做家庭随访

Nurse: How are you Mr. You? We come to see how you are doing. Have you been feeling okay after leaving the hospital? And, how is your blood gulcose level   control?

Patient: Very well. After leaving the hospital. I have been going back to the clinic every week and have my glucose checked. My blood glucose level has stabilized in the normal range. You see , the fasting glucose was 7-8 mmol/L. I also measured the blood guucose two hours after meal for two times. Not too bad, below 11mmol/L

Nurse: Great. Was your blood glucose level always that good?

Patient: I have not measured it for more than two months, because i feel very good . I am not thirsty, nor tired. i think it should be normal. And  i don't take teh medication anymore.

Nurse: Why? You were doing so well before and you know the harm of high blood glucose level!
